However custody documentation of fire debris evidence is required for both
receiving samples for testing and releasing samples back to the certified fire investigator.
If you have a browser that supports Microsoft Word, then the forms identified
as MS-WORD versions may be completed online and printed so that they may
accompany your samples. The PDF versions are present for easily printing
blank forms.
